A blue-faced angel with a worm

I got my blue face angel about 1.5 months ago, scored a 4.5. Had a rocky start in quarantine - ich outbreak 1 week in. Treated with hypo salinity for 5 weeks, then moved to display tank a week ago. Next day, noticed a scale out of place, thought maybe tank mate issues (but suspected external parasite by day 3). Today, day 7, first dislocated scale fell off, and I'm seeing a parasite/worm thing sticking out. Pics attached (http/www.flickr.com/photos/48448315@N04/?saved=1). Help with diagnosis and treatment would be awesome. Fish is still active, eating, and seeming happy.

My display tank's a 180g with 40g wet/dry, 30gallon refugium, 6in deep sand bed.
- PH 8.1
- Ammonia 0
- Nitrite 0.1 (water supply issues, normally 0)
- Nitrate >20
- Phosphates 0
Tank mates: 13in tesselata eel, 7in porcupine puffer, 5in Yellow tail Red Coris Wrasse, 2.5in dog face, 2in maroon clown, 1 watchman gobi, 1 sand sifter gobi (don't like lol), 2.5in male lyretail anthias, 1in female lyretail, 1in blue damsel, 1in flame angel
Food: frozen Market shrimp, pe mysis shrimp, spirulina brine shrimp, dried sea weeds, formula 2 marine pellets
