Cloudy eyes can also be from poor water quality over time. Even if params look good now, maybe previous spikes caused it. How often do you do water changes?

I'd second the meds if salt doesn't work. Kanaplex is solid. Also, maybe bump the temp to 80F, helps their immune system. Just don't go higher, they'll cook.

Had a similar scare once. Turned out the fish was just old. How long you had this guy? Sometimes it's just age.

Check the expiration date on your food. Old pellets can lose nutrients, leading to health issues. Happened to my buddy's betta.

Might be worth isolating him if you have other fish. Don't wanna risk spreading anything. You got a quarantine tank?
