I'm no help with the ID, but I've come across a few warnings that epoxy can irritate the coral if it's in direct contact with the flesh. Don't own a brain coral myself, just passing on what I've read.

Just picked up this awesome open brain at the LFS for a steal - 45 bucks. He's instantly my new favorite in the tank. Now I'm just trying to figure out what he actually is. The store called him a "metallic dome brain," but I couldn't find much info on that.

I think it's worth considering the timeline of when all the fish and the coflake were added. Was it all done in a relatively short period? I'm inclined to agree with the notion that a sudden spike in bioload could've triggered the water parameter changes. As for how quickly the fish were affected ...

Make sure your water parameters are on point, as any fluctuations could be causing your torch's unusual appearance.

You might want to look into how the cardinal was caught, maybe it was cyanide fishing, and get some info from your LFS about where the fish came from.
