Torch coral decline: causes and solutions

Hey guys, so I just got this torch coral about a week ago and it was doing great at first. I added a new power head for a week, but I just removed it 'cause the flow was too strong. The main head was getting blasted by the flow and I think that's what killed it. The other head seems fine though. Things started going downhill a couple days after I fed my corals some phyto and zooplankton. My water params have been solid, so I'm hoping the other head makes it. This torch is one of the tallest things in my tank, but it's still a foot deep. I've got 2 48" LEDs with white and actinic blue lights over the whole 125 gallon tank, and a separate light over the torch. The tentacles on the dying head have been falling off and shrinking over the past couple days - they go from brownish tan to almost black before disappearing. Any ideas from experienced LPS keepers would be awesome, Happy reefing
