A couple of things to check: are the scales protruding from the sides of its body? Is it eating as usual? What about its poop - is it looking normal?

If it's stopped eating and its scales are sticking out, that's a bad sign - it might have an internal bacterial infection, also known as dropsy, and unfortunately, that's usually fatal.
It's kinda weird, I don't usually look at my fish from the top, but from the side it looks normal.

It's eating fine, no issues there.

The water's clear, no cloudiness or anything.

I've got a waterfall filter on the tank, and to be honest, I don't clean it that often. I'm worried about messing up the bacteria in the substrate, so I only clean it when it gets clogged with algae and poop.

You've got this, it's manageable.
