Back to the Nitrate issue, I'm running a canister filter with sponge and ceramic media. It's been about 6 months since I last cleaned it. Maybe that's part of the problem.

Definitely give that canister a clean. A clogged filter can't do its job properly. Just rinse the media in tank water, not tap, to preserve the beneficial bacteria.
