I thought it was something really bad, his insides coming out or something.

Is it protruding from his anus? If so, it's likely a prolapse, which is a serious condition. You can give it some time and see if it retracts on its own, but unfortunately, that's rarely the case.

I'm not sure, but it seems attached near his fins and it's really affecting his swimming. It's all tangled up in his fins underneath, which is concerning.

Can you get a clearer pic for me? I'm not convinced it's a tumor, not with it moving around like that and getting tangled in his fins.
